Application of CT perfusion imaging in evaluation the changes of skeletal muscle perfusion before and after revascularization of lower limbs

Abstract:

Objective

To evaluate the skeletal muscle perfusion using CT perfusion imaging before and after interventional treatment in patients with lower extremity arteriosclerosis obliterans.

Methods

18 patients with lower extremity arteriosclerosis obliterans were included in the study. Endovascular treatment was performed according to standard procedures in one lower extremity. The other one without endovascular treatment was included in the control group. CT perfusion scans were performed 3 days before and after treatment to collect all image data. After the CT perfusion image was processed by VPCT Body software, the bilateral medial plantar muscles were selected symmetrically as the region of interest (ROI), and the perfusion parameters were obtained: blood flow (BF), blood volume (BV), and mean transit time (MTT) and permeability (PMB). Wilcoxon signed rank test was used to compare differences between perfusion parameters.

Results

18 patients were successfully treated with endovascular treatment. Before interventional therapy, there was no significant difference between BV, BF, PMB and MTT in the treatment group and the control group. After interventional therapy, the BV, BF and PMB in the treatment group were significantly higher than those in the control group, and MTT in the treatment group were significantly lower than that in the control group. Before and after interventional therapy, there were no significant differences in BV, BF, PMB and MTT in the control group. In the treatment group,BV, BF and PMB were significantly higher , and the MTT decreased significantly.

Conclusions

CT perfusion imaging can be used to measure and evaluate the change of perfusion in lower extremity skeletal muscle in real time, which has high clinical value.
