目的

观察介入置管行臭氧水低压灌注冲洗治疗胸/腹腔感染合并外瘘形成患者的临床疗效。

方法

16例因手术或外伤导致胸/腹腔感染合并外瘘形成的患者,其中14例检出1~4种细菌,在DSA引导下介入置管行瘘道臭氧水低压灌注持续冲洗,部分患者联合抗生素治疗,观察患者炎性指标变化及临床治疗效果。

结果

16例患者住院期间7例治愈,8例临床症状好转,1例因炎症侵蚀大血管并发大出血死亡。治疗前后白细胞、中性粒细胞、C反应蛋白、降钙素原值均有不同程度下降(P值均<0.05)。抗生素使用天数占总住院天数的31.56%,抗生素费用占治疗费用比为5.72%。

结论

DSA引导下介入置管行胸/腹腔感染合并外瘘窦道臭氧水冲洗治疗,有一定的临床及社会价值,但治疗方法有待于进一步研究。

Objective

To observe the clinical efficacy of low-pressure ozone water flushing with interventional catheterization in the treatment of patients with intra-thoracic/intra-abdominal infection combined with external fistula.

Methods

A total of 16 patients with intra-thoracic/intra-abdominal infection combined with external fistula caused by surgery or trauma were selected. Among them, 1-4 species of bacteria were detected in 14 patients. Under the guidance of DSA, the sinus was treated with persistent low-pressure ozone water flushing with interventional catheterization, and some patients were additionally treated with antibiotics. The changes in inflammatory indexes and clinical efficacy were observed.

Results

Among the 16 patients, 7 were cured, 8 improved and 1 died of massive hemorrhage due to inflammation eroding large blood vessels during hospitalization. After treatment, leukocytes, neutrophils, C-reactive protein and procalcitonin decreased in varying degrees compared with those before treatment (P < 0.05). The time of using antibiotics accounted for 31.56% of the total hospital stay, and the cost of antibiotics accounted for 5.72% of the treatment cost.

Conclusions

DSA-guided ozone water flushing with interventional catheterization for the sinus in intra-thoracic/intra-abdominal infection combined with external fistula has certain clinical and social value. The treatment method needs to be further studied.
