目的

探讨经股静脉置入下腔静脉滤器回收钩方向与取出时股静脉入路选择对抓捕次数的影响。

方法

纳入2023年7月至2025年3月无锡市惠山区中医医院患下肢深静脉血栓伴或不伴有肺动脉栓塞的患者26例,经股静脉途经下腔静脉滤器置入,抗凝取栓后取出下腔静脉滤器。其中经回收钩开口方向对侧股静脉入路回收滤器9例,同侧入路17例。取出滤器后,口服抗凝治疗6个月并定期复查。

结果

26例患者均成功取出滤器。经回收钩开口方向对侧股静脉入路回收滤器9例,抓捕7~12次,同侧入路17例,抓捕1~4次,差异有统计学意义(P<0.001)。

结论

经股静脉入路置入滤器回收时选择开口方向同侧股静脉入路,能明显减少抓捕次数,可为经股静脉回收梭形滤器时入路选择提供参考。

Objective

Investigating the impact of the retrieval hook orientation of inferior vena cava filters implanted via the femoral vein and the choice of femoral vein approach during retrieval on the number of capture attempts.

Methods

26 patients with lower limb deep vein thrombosis with or without pulmonary embolism were treated by inserting an inferior vena cava filter through the femoral vein. After anticoagulation and thrombectomy, the inferior vena cava filter was removed. Among them, there were 9 cases of retrieving filters through the contralateral femoral vein approach with the opening direction of the retrieval hook, and 17 cases through the ipsilateral approach. Following the removal of the filter, the patients underwent oral anticoagulant therapy for a duration of six months and were subjected to regular follow-up examinations.

Results

26 patients successfully removed the filters. Nine cases were retrieved from the contralateral femoral vein via the retrieval hook opening direction, with 7-12 captures, while 17 cases were retrieved from the ipsilateral approach, with 1-4 captures. The difference was statistically significant (P<0.001).

Conclusion

When retrieving a filter via the femoral venous approach, selecting the same side as the opening direction of the femoral venous access can significantly reduce the number of captures, making. It can serve as a reference for selecting the approach when retrieving the biconical inferior vena cava filter via the femoral vein.
