地佐辛在肝癌射频消融中的镇痛效果研究

Analgesic effect of dezocine in radio frequency ablation for treatment of hepatic carcinoma

目的:

研究地佐辛在肝癌射频消融(RFA)术中的镇痛效果。

方法:

选取2014年1月—2016年10月我科收治的60例行RFA治疗的肝癌患者,随机分为两组。A组术前肌注哌替啶75 mg。B组术前肌注地佐辛0.1 mg/kg,术中静脉泵入地佐辛0.5 mg/kg·h。比较两组的RFA完成情况、术中术后视觉模拟评分(VAS)、患者满意度评分及不良反应发生率。

结果:

A组28例、B组30例不间断完成RFA手术。术中B组的VAS评分显著低于A组(3.3±1.7 vs. 5.9±1.4,P<0.05),术后B组的VAS评分显著低于A组(2.3±1.4 vs. 4.6±1.3,P<0.05)。B组满意度评分(1分:非常好、2分:好、3分:一般、4分:不好)分别为8、13、7、1例,A组分别为3、10、12、5例,B组患者术后的满意度评分较好者的比例高于A组,差异有统计学意义(P<0.05)。B组不良反应发生率显著低于A组(6.7% vs. 16.7%,P<0.05)。

结论:

采用地佐辛镇痛可以保障肝癌RFA手术的完成,其效果优于哌替啶。

Objective:

To investigate the analgestic effect of dezocine in radio frequency ablation (RFA) for the treatment of hepatic carcinoma.

Methods:

Sixty patients with hepatic carcinoma were enrolled in this study. These patients were randomly divided into two groups: Group A (n=28) and Group B (n=30) . Before operation, the patients in Group A were IM given pethidine (75 mg) while the patients in Group B were treated with dezocine (0.1 mg/kg) . The visual analogue scale (VAS) and patients satisfaction scale were used to evaluate the comfort degree, and the incidence rates of adverse drug reactions were also recorded.

Results:

In the operation, the VAS score in Group B was lower than that of the Group A (3.3±1.7 vs. 5.9±1.4) and the difference was statistically significant (P<0.05) . After operation, the VAS score in Group B was lower than that of Group A (2.3±1.4 vs. 4.6±1.3) and the difference was statistically significant (P<0.05) . Satisfaction level (1:very good, 2:good, 3:general, 4:bad) in group B (8, 13, 7, 1) was better than that in group A (3, 10, 12, 5) and the difference was statistically significant (P<0.05) .

Conclusions:

The use of dezocine can ensure the completion of RFA operation for HCC, and the effect is better than that of pethidine.
