输尿管结石应用64排容积CT尿路成像的诊断价值

目的

探讨64排容积CT尿路成像在输尿管结石中的诊断价值。

方法

分析200例输尿管结石的患者,其被临床或者手术判定为输尿管结石,所有入选的患者行64排容积CT尿路成像,并采用不同图像后处理:多平面重组法、曲面重组、最大密度投影、容积再现、表面成像。

结果

在入选的200例患者中,结石最小的直径为1.5mm,结石位于输尿管上段的有70例,位于下段的有83例,位于中段的有35例,位于输尿管上中下段的有12例。同时还发现合并输尿管畸形的患者。其中,合并肾囊肿的有100例,输尿管囊肿的有5例,膀胱憩室的4例,合并肾结石的有30例,合并膀胱结石的有10例,合并输尿管及肾积脓的2例,肾盂及输尿管畸形的有20例。

结论

64排容积CT对于输尿管结石的定位、大小、形态、数量及其周围情况的诊断具有重要的价值,同时可显示泌尿系统是否合并畸形及肾结石、膀胱结石。

Objective

To evaluate the diagnostic value of 64-slice volume CT scansfor detecting ureteralstones.

Methods

Clinical data of 200 patients with ureteral stones were reviewed and analyzed. All the enrolled patients underwent 64-slice volume CT scanningand the images were analyzed including multi-planar reconstruction, curved planar reformation, maximum intensity projection, volume rendering, and surface shade display. All patients had complete imaging data.

Results

Among the enrolled 200 patients, the diameter of the smallest stone was 1.5mm. The stones were located in the upper ureter in 70 cases, in the middle ureter in 35 cases, in the lower ureter in 83 cases, and multiple calculi in the upper, middle and lower segments of the ureter in 12 cases. It was also found some other accompanyingchanges: renal cysts in 100 cases, ureterocelein 5 cases, bladder diverticulum in 4 cases, renal calculi in 30 cases, bladder stones in 10 cases, empyema of the ureter and renal pelvis in 2 cases, and ureter and renal pelvis malformations in 20 cases.

Conclusions

64-slice volume CT has important significance for the diagnosis of ureteral calculi, their localization, size, morphology, amount and surrounding circumstances, and also to reveal the accompanying alterations such as urinary malformations, and renal or bladder stones.
