目的

探究超声引导下微波消融治疗下肢静脉曲张的临床疗效。

方法

选取60例下肢静脉曲张患者(共80条肢体),其中接受传统手术(高位结扎加点式抽剥术)治疗的30例(40条肢体)为对照组,接受超声引导下微波消融术治疗的30例(40条肢体)为研究组。记录两组患者临床效果、手术相关指标、治疗前后静脉临床严重程度评分(venous clinical severity score,VCSS)、慢性静脉功能不全问卷(chronic venous in-s insufficiency questionnaire,CIVIQ)、治疗满意度和并发症。

结果

研究组的治疗总有效率(96.7%)显著高于对照组(73.3%) (P < 0.05);研究组的手术时间、住院时间及术中出血量明显短/少于对照组(均P < 0.05),但手术费用高于对照组;与术前相比,两组患者术后6个月的VCSS评分均明显降低,CIVIQ评分明显提高;研究组术后3、6个月的AVVSS评分低于对照组,而CIVIQ评分高于对照组(均P < 0.05);研究组的治疗满意度为(96.7%)明显高于对照组(73.3%) (P < 0.05);研究组的并发症发生率(6.7%)显著低于对照组(33.3%) (均P < 0.05)。

结论

超声引导下微波消融术和传统手术治疗下肢静脉曲张均一定的疗效,但超声引导下微波消融治疗的疗效更好,具有微创、简便、疗程短、生活质量高及并发症少等优势。

Objective

To explore the clinical efficacy of ultrasound-guided microwave ablation for lower extremity varicose veins.

Methods

Sixty patients (a total of 80 limbs) with lower extremity varicose veins were selected, of which 30 cases (40 limbs) treated with traditional surgery (high ligation and stripping) were the control group, and 30 cases (40 limbs) treated with ultrasound-guided microwave ablation were the study group. Clinical efficacy, surgical-related indicators, venous clinical severity score (VCSS), chronic venous insufficiency questionnaire (CIVIQ), treatment satisfaction, and complications were recorded for both groups.

Results

The total effective rate of the study group was significantly higher than that of the control group (96.7% vs 73.3%, P < 0.05). The operation time, hospitalization time and intraoperative blood loss in the study group were significantly shorter/less than those in the control group (all P < 0.05), but the operation cost was higher than that in the control group. Compared with before treatment, the VCSS scores of both groups significantly decreased, and the CIVIQ scores significantly increased 6 months after operation. The AVVSS score of the study group was lower than that of the control group at 3 and 6 months after operation, while the CIVIQ score of the study group was higher than that of the control group (all P < 0.05). The treatment satisfaction rate of the study group was significantly higher than that of the control group (96.7% vs 73.3%, P < 0.05). The incidence of complications in the study group was significantly lower than that in the control group (6.7% vs 33.3%, P < 0.05).

Conclusions

Both ultrasound-guided microwave ablation and traditional surgery had certain therapeutic effect on lower extremity varicose veins, but ultrasound-guided microwave ablation has better efficacy, with advantages of minimally invasive, simple, shorter treatment duration, higher quality of life, and fewer complications.
